Rolexan®

Generic Name: Enoxaparin Sodium

Form: Pre-filled syringe containing 0.4 and 0.6 mL of injectable solution (100 mg/mL)

Drug Class: Anticoagulant

Indications: Rolexan® is an anticoagulant used for the prevention and treatment of venous thromboembolism.

 

Rolexan®

Rolexan® (Enoxaparin Sodium) is a low-molecular-weight heparin that works as an anticoagulant by inhibiting factor Xa, preventing blood clotting.

Indications for Rolexan®

  • Ischemic Heart Disease

  • Treatment of Superficial Vein Thrombosis (SVT)

  • Prevention and treatment of Venous Thromboembolism (VTE)

  • Frostbite

  • Anticoagulant for patients undergoing intermittent hemodialysis

Dosage Form

  1. Pre-filled syringe containing 0.4 mL of injectable solution (100 mg/mL)

  2. Pre-filled syringe containing 0.6 mL of injectable solution (100 mg/mL)

Preparation of Rolexan®

Rolexan® is available as a pre-filled syringe and requires no preparation.

Dosage and Administration

  • Rolexan® is administered subcutaneously. It can be intravenously administered under the supervision of a healthcare provider if necessary.

  • It should not be injected intramuscularly.

  • Each syringe is for single use only.

  • Do not expel the air bubble from the syringe before injection as it helps with proper administration.

  • Do not remove the syringe cap until you are ready to inject.

  • The solution is colorless or slightly yellow. If there is any discoloration, cloudiness, or foreign particles, do not use.

Injection Procedure

  1. Wash hands and clean the injection site with soap and water.

  2. Sit or lie in a comfortable position to ensure access to the injection site.

  3. Select the injection site on the abdomen, at least 5 cm from the navel.

  4. Remove the syringe cap, being careful not to contaminate the needle.

  5. Pinch the skin between your thumb and index finger to create a raised area.

  6. Hold the syringe at a 90-degree angle and insert the needle into the raised skin.

  7. Push the plunger with your thumb to inject the solution, keeping the skin pinched.

  8. After injecting, hold the syringe for a few seconds before withdrawing it.

  9. Replace the cap on the syringe and dispose of it properly.

Monitoring During Treatment

Monitor platelet count, hemoglobin, hematocrit, occult blood in stool, signs of bleeding, serum creatinine, and Xa activity if necessary.

Contraindications for Rolexan®

  • History of hypersensitivity to Enoxaparin, Heparin, or any components of the formulation.

  • History of heparin-induced thrombocytopenia (HIT) within the past 100 days or presence of circulating antibodies.

  • Active bleeding.

  • Severe bleeding disorders.

  • Conditions that predispose to bleeding (e.g., active gastric or duodenal ulcers, uncontrolled hypertension, diabetic or hemorrhagic retinopathy, brain, spinal cord, eye, or ear surgeries or injuries).

Warnings and Precautions

  • Risk of epidural or spinal bleeding in patients receiving local anesthesia or spinal needle insertion.

  • Monitor for signs of bleeding, such as unexplained weakness, hypotension, dark stools, or blood in the urine.

  • May increase serum potassium in patients with kidney dysfunction or those on potassium-sparing diuretics or supplements.

  • Use cautiously in elderly patients or those with renal failure.

  • Higher bleeding risk in patients with low body weight (less than 45 kg in women, less than 57 kg in men).

  • Dose adjustments may be needed for patients with obesity (BMI over 30).

  • Do not substitute for heparin or other low-molecular-weight heparins.

Use of Rolexan® in Pregnancy and Lactation

Rolexan® can be used during pregnancy and lactation under the supervision of a physician.

Side Effects of Rolexan®

While this drug has therapeutic benefits, it may cause side effects in some individuals, including:

  • Nausea

  • Anemia

  • Injection site reactions

  • Fever

  • Bruising

  • Peripheral edema

  • Hematuria (blood in urine)

  • Dizziness

  • Elevated liver enzymes

Consult with a healthcare provider or pharmacist before using Rolexan® with other medications or supplements.

Packaging

Each package of Rolexan® 6000 IU contains 2 pre-filled syringes, and each package of Rolexan® 4000 IU contains 6 pre-filled syringes.

Storage

  • Store below 25°C, away from light, moisture, and freezing.

  • Do not use beyond the expiration date.

  • Keep out of reach of children.

  • Store in the original packaging until use.
